Enrollment forms

Requirements for enrollment:

• Parent-Provider Contract Agreement
• Emergency Contact Form
• Most recent physical and immunization record (CT Health Assessment Record)
• Diaper Cream Authorization Form (if applicable) 

Everyone must read the Parent Handbook before signing agreement form. This handbook includes all state required 

policies including Behavior Management and Sick Policies. 

**The Authorization of Medication Form is only required if your child must be given prescription medication at daycare.**
